2. 本系獎學金申請提交表單（附件2）。
3. 歷年學業成績單正本。
4. 學生證正反面影本。
5. 指導教授推薦函。
6. 其它有利申請之資料，如研究成果等。

※ Recipients recommended by the department must submit the required hard-copy documents to the person in charge within the specified period. Late submissions will be considered as forfeiting the award.

3. Original Transcript of Academic Record.
4. Photocopy of the front and back of the student ID card.
5. Recommendation letter from the advisor.
6. Other supporting documents, such as research achievements.
