※ 申請人請填寫附件4之獎助學金申請提交表單email至承辦人員電子信箱chiench@ntu.edu.tw，並請將申請文件依下述順序順序排列，於截止日期前送交至化工二館N210室。
1. 申請表（附件1。申請表之未得其它獎學金證明欄位，由系辦審查核章）。
2. 切結書（附件2）。
3. 前一學年成績單正本。
4. 前一學年操行成績單正本。
5. 戶口名簿影本或全戶戶籍謄本。
6. 專題報告1篇。
7. 著作目錄（附件3），請email至承辦人員電子信箱chiench@ntu.edu.tw，若無則免。
8. 著作目錄所列各篇著作之第1頁，若無則免。
※ Applicants are required to fill out the Scholarship Submission Form (Attachment 4) and email it to the responsible staff member at chiench@ntu.edu.tw. Please arrange the application documents in the following order and submit them to Room N210, Chemical Engineering Building II, before the deadline.
1. Official Application Form (Attachment 1. The section for proof of not receiving other scholarships must be reviewed and stamped by the department office).
2. Declaration Form (Attachment 2).
3. Original academic transcripts for the previous academic year.
4. Original conduct records for the previous academic year.
5. Photocopy of household registration or full household transcript. One research report.
6. One research report.
7. List of publications (Attachment 3), please email it to the responsible staff at chiench@ntu.edu.tw; if none, this can be omitted.
8. The first page of each publication listed in the list of publications; if none, this can be omitted.
