2. 切結書（附件2）。
3. 本系獎助學金申請提交表單（如附件3，請上傳Excel檔）。
4. 歷年學業成績單正本。
5. 操行紀錄證明。
6. 戶口名簿影本或全戶戶籍謄本。
7. 專題報告1篇。
8. 期刊論文著作各篇之第1頁，若無則免。
9. 其他有利審查資料著作目錄所列各篇著作之第1頁，若無則免。
