1. 校內用申請書（如附件1）。
2. 歷年成績表。
3. 獎懲紀錄證明書。
4. 操行成績證明書。
5. 戶籍謄本。
6. 家境清寒證明文件，例如：政府開立之低收入戶、中低收入戶、特殊境遇證明、家中發生變故，或其他相關證明如助學貸款等。並需檢附全家人前一年國稅局或稅捐處所得資料清單正本或有國稅局浮水印之影本。
7. 本系獎助學金申請提交表單（如附件2，請上傳Excel檔）。

1. Application form (Attachment 1).
2. Academic transcripts for all semester.
3. Reward and punishment record certificate.
4. Conduct certificate.
5. Household registration transcript.
6. Documentation proving financial hardship, such as government-issued certificates for low-income households, middle-low income households, or special circumstances, as well as evidence of any family emergencies or related documentation such as student loans. Additionally, include the original or a photocopy with the National Tax Bureau watermark of the income information list for all family members from the previous year.
7. ChemE Scholarship Submission Form (Attachment 6, please submit as an Excel file).
