※ 申請人請填寫附件3之獎學金申請提交表單email至承辦人員電子信箱nichenlee@ntu.edu.tw，並請將申請文件依下述順序排列，於截止日期前送交至化工二館N204室。
1. 申請書（附件2）。
2. 歷年成績單正本。
3. 歷年名次證明正本。
4. 歷年操行成績單。
5. 歷年獎懲紀錄證明。
6. 政府低收或中低收清寒證明正本（如無者則一律檢附全家人前一年國稅局或稅捐處所得資料清單正本或有國稅局浮水印之影本）。
※ Applicants are required to fill out the Scholarship Submission Form (Attachment 3) and email it to the responsible staff member at nichenlee@ntu.edu.tw. Please arrange the application documents in the following order and submit them to Room N204, Chemical Engineering Building II, before the deadline.
1. Application Form (Attachment 2).
2. Original official transcripts from all previous academic years.
3. Original official class ranking certificates from all previous academic years.
4. Conduct records from all previous academic years.
5. Certificates of rewards and disciplinary records from all previous academic years.
6. Original certificate of low-income or economically disadvantaged status issued by the government (if unavailable, attach the original income statement for the previous year for the entire family from the National Taxation Bureau or a photocopy with the watermark from the National Taxation Bureau).
